Can anyone tell me why my truck is starting to sag in the rear drivers side? What should I look at or do to correct this?

check the bushings.

could have a cracked leaf, rotten bushings, broken body mount....
hard to say

Get under there and look? Like brokentoy said, there are plenty of things it could be, they're all no-brainers.

Heck even a sagged spring could be at fault.....my car (yeah I know a car isn't a truck but the theory applies) was nearly sitting on it'e left rear tire....was simply the rear coils...put a new pair in and she's been sitting perfectly ever since. And if the truck is the same one that's in your photo with your sig line, it's a good idea to check the spring....your gas tank is on that side of the truck.

Its a twisted spring... need to straighten it out and it will be fine.
